IEP/504 Toolkit Downloads

1. Pre-Meeting Preparation Checklist Help parents get organized and confident before an IEP or 504 meeting.

Includes:

  • ☐ Review latest IEP/504 Plan

  • ☐ Gather evaluation reports or outside diagnoses

  • ☐ Draft parent input statement (strengths, concerns, goals)

  • ☐ Prepare a list of questions or requests

  • ☐ Print/email any supporting documents

  • ☐ Know who will attend the meeting

  • ☐ Confirm date/time and request copy of draft plan (if possible)

  • ☐ Understand key rights (FAPE, LRE, PWN, etc.)

2. Parent Input Builder Worksheet Guide parents in writing a clear, strengths-based statement for meetings.
